This study is designed to test Self-Determination Theory for its potential use in improving health outcomes. The six-month intervention will target smoking cessation and low fat diets for those smokers with elevated lipids. The intervention will support patients autonomy, competence and relatedness needs to improve participant motivation and skills specific to the focal health risk behaviors. The 1,400 participants will include hospitalized inpatients and individuals contacted through physicians' offices, their insurers, or cooperating work-sites.
